Application In Order To Start The Pre-Approval Process. Don't worry if
there are a few blanks that don't apply. Fill out as much as possible
and when we speak, we will make sure we have all the information needed.

| How To Get A Duplicate Copy Of Form DD214
Your
DD214 form is required in order to obtain your certificate of
eligibility for a VA mortgage loan. If you have misplaced your
original DD214 form you can request another copy by completing and
submitting Form SF 180 "Request Pertaining To Military Records." You
can download Form SF 180 by Clicking Here. You
can also request dd214 duplicate copies by using the VA fax on demand
system. Just dial 301-837-0990 from a fax machine. Follow the voice
instructions and when prompted for the document number enter 2255.
The DD-214 can now be requested online, but ONLY BY:
- Next of kin of a deceased, former member of the military.
The
next of kin can be any of the following: surviving spouse that has not
remarried, father, mother, son, daughter, sister, or brother. Persons
fitting one of these two categories can request military records online
from the eVetRecs Website by Clicking Here.
Be sure to read all instructions before beginning the request process.

Costs and Response Time
"Generally
there is no charge for military personnel and health record information
provided to veterans, next-of-kin, and authorized representatives. If
your request involves a service fee, you will be notified as soon as
that determination is made. Response time varies dependent upon the
complexity of your request, the availability of records, and our
workload. Please do not send a follow-up request before 90 days have
elapsed as it may cause further delays."

VA Administration, and within a few weeks you will receive a
Certificate of Eligibility (green colored) in the mail from the VA. We
